Most of the forms can be filed out by keyboard but must be signed with a pen. We request that forms be returned by the first day of school, sooner if possible.
Forms for new students and those entering 5th grade:
- Media Release (New students only.)
- Health Info Exam (New students and 5th grade only.)
- Dental Exam (New students and 5th grade only.)
- School Records Request (New transferring students only.)
- We must have a copy of each student’s birth certificate on file and a copy of court document concerning custody if applicable.
